What to expect when you arrive for your appointment

Please arrive 15 minutes prior to your scheduled appointment time to complete our new patient paperwork. If possible, we request completing our online Patient Forms prior to your appointment to expedite the check-in process. These forms are required to be updated yearly.

Please be sure to bring your insurance cards, photo identification, and a list of current medications. Pertinent records from a prior office are beneficial. If your insurance has changed, it is important to notify us prior to the appointment to clarify benefits. We recommend wearing no make-up and dressing in garments that easily reveal the area in question.

Dr. Haynes and her associates are dedicated to providing the highest level of care. We also strive to be timely with appointments. Following the recommendations listed above is greatly appreciated to help keep us on schedule. If you cannot make your appointment please notify us as soon as possible so we can offer it to another patient. We will be happy to accommodate an appointment change.

New patients must pay a $75 cosmetic consultation fee, which can be applied toward procedures and products at Skin Wellness Center. Should you have any questions prior to your appointment date, please feel free to contact our office at (727) 545-3376. We look forward to seeing you soon.

Cancellations

Skin Wellness Center had adopted a no-show policy effective July 17, 2019. We will now charge $25 for each missed “no-show” appointment, and cancellations made with less than 24 hours notice. This policy applies to new and established patients and will be charged directly to the patient/guarantor, NOT to the insurance. All no-show fees MUST be paid prior to the next appointment in order to be seen. We reserves the right to terminate the doctor-patient relationship of established patients due to no-shows. We thank you for your cooperation in this matter.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
